What would it take.. (cost-wise, and equipment-wise) to construct my very own, personal ISP?

Depends. For dial up, I imagine it'd be possible with a webserver with a modem and a phone line. Of course only you could use it.. making it kinda pointless. It'd cost probably 1000x more than just using an existing isp :p

Well it all depends. If you want to support dialup users, you'd need a modem bank, one to handle each user similtaneously connected. Then to actually connect to the internet you'd need to get bandwidth from a high level provider and setup a network infrastructure to get your users connected to that rented line. Then there would be further complications if you wanted to offer DSL or cable service.
