I didn't either. Just do the following:

Pick a mode (realistic or casual, though I've yet to notice a significant difference)

Pick virus, bacteria or the other thing (you get different strengths and weaknesses when starting off).

That should bring you to a world map, where it says your disease has started. To really get infecting, you have to click on "disease" and upgrade your symptoms and resistances and such.

It's sort of like an RPG. If I could pick up on it, you all can.

Well, I've killed 1.83 billion so far, drug resistance is maxed out so they're taking a while on the vaccine but the flow of evolution points have slowed right down and I can't afford the new tier of horrific symptoms for a while.

Wooo and as I type this I turned New Zealand and Canada into forsaken regions. Silly relaxed friendly people, look where it got you now! Bahahahaha

EDIT: It fizzled out after I also killed off the populations of the USA, Mexico, India and Japan and hadn't managed to get into any other countries. I think you need to jack up your communicability and resistance as quietly as possible and then ramp up the deadliness.

I scored 48705 on casual mode with bacteria.

I used a bacteria to wipe out a whole lot of the earth's population. Unfortunately I couldn't seem to kill the USA, Mexico, or half of South America.
Ended up with a score of 86000 or so. Good ol' "Fred" was the disease :)

Had another go with a virus and simply couldn't get into Madagascar or Greenland. Killed the other 6.3 billion people though and scored 157280.

Madagascar is a pain from everyone I've heard (including myself). If you don't have your disease start there, you best hope it sneaks there before your visibility goes too high up. Otherwise, they lock down and it's nearly impossible to get them infected.

Same goes for a number of island places I suppose, but them in particular it would appear.

Short Strategy:

Start in Madagascar.

Long Strategy:

Parasite. Start in a heavily-port-driven nation like New Zealand, Greenland, or Madagascar. Remove any symptoms. Up all the resistances 2 except drugs. Then up the drugs to 5, and lastly up the water. Hope you nab Madagascar. Restart if any of the nations fully shut down. Once you're in every nation, get all of the most painful deadly symptoms. Crack one open and watch the world die.

I always do a virus, I've killed off everyone except Japan, Mada, and Cuba. Somehow the stupid islands get word fast.

Haha, victory! I wanted to cheer when I saw "Madagascar completely devoid of life." Things went to hell fast when I added hemorrhaging to a previously symptomless disease.

You managed to kill the world with nothing but Tier I symptoms? Wow.

And damn. If you guys think it's tough trying to get a disease into Madagascar, try getting one out of Madagascar. One month and counting, and still no other countries infected.

Edit: Yep, Madagascar is a trap. By the time your disease gets out of the country, it's infected enough people in Madagascar to get the rest of the world paying attention.

hehe, I'm playing now and I got the entire world except India, Madagascar and greenland infected, all the ports are closed, and there's this little ship bouncing back and forth from port to port cause noone will let him in!

i rule! finally got greenland and madagascar, to rule the world with my virus "Disease name". interestingly, i had no resistances until late game. just went straight to tier 4 and got heart failure, etc, after getting the transmission advantages.

killed the whole world last night after an hour and a half of playing and slowly gathering points. I think virus is probably the hardest one to play because as soon as it spreads to other countries everyone else is like "OMFG" -closes-.

Parasite FTW.

Started as a fever. I just let it gather points in the cuba, where it originated, then slowly added the transmissions, and the level 1 resistances, then opened up the tiers til tier 4. Let it spread to everywhere in the world posing as a mere fever, then boom, bought heart and liver failure and everyone died. I just got a really lousy final score of like 60k-something.

I think I got major lucky though, because they invented and distributed the vaccine, but it was a dud and caused my disease to mutate.

Trying a new strategy. Using bacteria, NO symptoms, only transmissions (and water resistance II so I can infect water supplies) until I infect every country.

Then hemorraging.

I can get all but one or two countries with a virus -- just skip resistances and focus on transmissions and "infectivity" and let it grow as fast as possible. It's more of a blitzkrieg approach than the parasites. The trouble is that unless the timing's perfect you won't get your virus onto a Madagascar-bound ship before they close the ports. Parasites allow more time for that to happen.

Well, I infected every country, but then the vaccine worked, so I tried again (as parasite this time) and got everyone but cuba and madi-f'in-gascar. I'm currently building up points to get hemmoraging and heart attacks, but I've essentially won.

Selling your first symptom is a good idea. I've been able to infect all, or all but one or two countries, every single time. Usually nothing happens for a while while you build up your resistances, and then BAM everyone gets infected instantly.

Also, does anyone get a weird sensation from the back of your head time to time, as you comfortably watch numbers and colors fluctuate on your monitor, that forces you to consider, for just a second, the utter madness and blood-ridden chaos these countries must face as your disease slowly gains strength? Governments burning dead bodies, people violently hemorrhaging, children getting covered in open sores and so forth. Talk about literal hell on earth. It just makes you think, as the body count statistic rises into the billions, that, maybe one day, the human race will actually face such a threat. Imagine the intense feelings of hopelessness and despair as everyone you knew dying around you and the anger you have when you realize the future will be populated entirely by those damn Malagasian people.

Well, after about a half an hour I've infected the entire world but I've not had a single death because I have to wait for the dang points to gather.

It's an interesting game but it's more boring than the sims.

I finally destroyed humanity.
Went with parasite and was lucky enough to start in Madagascar. Had no symptoms but waited for all my resistances and transmissions before going lethal. By that time the whole world was infected. Then all that was left was to await sweet 0 humans :)
