And that wouldn't affect anyone outside the middle east? Like, no radiation spreading around or anything?

I'm pretty sure the fallout could and most likely would spread over a large part of Europe.

Low-yield fusion bombs can reduce radiation spread, but there is still plenty of radiation from the fission stage of the bomb.

It may be possible to use say, inertial confinement fusion to create a clean hydrogen bomb. At least maybe if the lasers for ICF weren't the size of warehouses.

The neutron flux is still enough to create radioisotopes.
