1) Take the first letter of your first name and average it with 'z'. This causes a back-heavy first (likely) consonant, which is more independent and masculine. For example: My name is Kevin so I take k (11) and average it with z (26). The result is (11+26)/2 which is 18.5. In this case, we round up so the result is 19—the letter 's'.
2) Take your first name minus the first new letter, so for example mine would be 'evin.' If this is composed of 4 or more letters, then take the first letter, e in this case, and move it to the end, forming 'vine.' If it's composed of 3 or fewer letters, then do the opposite, taking the last letter and moving it to the front.
3) Now tack on the letter you got from 1) to the front of the jumble of letters you got from 2). In my case, so far I have 'Svine.'
4) Take the first vowel in the result from 3) and use the following rule:
If it is a, change it to y.
If it is e, leave it alone.
If it is i, change it to u.
If it is o, leave it alone.
If it is u, change it to e.
If it is y, change it to i.
If there are no vowels, change the second letter to either o or e, depending on whether the letter is odd or even (refer to numeric list).
In my case with Svine, we change the i to a u according to the rule, causing Svune.
5) Change every consonant in the name to [consonant + 1]. With my name Svine this means we add one to s, getting t. Add one to v, getting w. And add one to n, getting o. If any of the results are NOT consonants, keep adding until you get one. Since we got o, we add one more and end up with p, so the name is Twipe instead of Twioe.
6) Reverse the order of the letters. Twipe becomes Epiwt.
7) This part is a little tricky. Take each pair of letters and reverse them. I reverse the 'Ep' in Epiwt and it becomes Peiwt. Then I reverse the 'iw' and we get Pewit. There's no pair to reverse with t, so we're done.
8) Remember the letter you got for 1)? Tack it onto the front again. We get Spewit. If this sounds disagreeable, you may alternately tack it onto the end or skip this step.
