I was only banned by accident once when basically a range of IPs got taken out and mine was in it, or some such like. Hence I've voted 'never' since officially I have never been banned for doing something bad.

I got ISP banned years ago when I was still using AOL and they banned all AOL IPs for a day or two. I can't remember all of the details surrounding it but quite a few members were affected. (Probably the same incident that Gee_4ce was referring to).
