Just finished my first playthrough as a Vanguard. Been avoiding this thread ever since I got the game a week ago, and I'm glad that I finally managed a game this epic without having something spoiled (usually I get too curious or read/hear something by accident). Will start my next game soon when I've settled on a class. Probably Sentinel.
I got utterly compulsive with the mining minigame until a fair way into the game when I realised that I had more resources than I'd ever need to spend. After that I just clicked on planets once to get every system up to 100% explored and to check for side missions. Not sure if that system worked out any better than the Mako from the first game, to be honest. It just doesn't seem like they considered just how much fun either option would be on subsequent playthroughs.
Yeah, the middle option (the one that wasn't "I am/am not interested") ended with Shepard saying something like "I'd never risk your safety like that whilst on a mission, no matter how interested I might be." I thought Tali was really well written during this part; it was a thought that came up in discussions of the first game about how a romance would've worked if they'd included Tali, and it always did strike me as odd, but her character carried over well.
The ammo system grated a little bit at first because I didn't understand how it differentiated the different weapons. I think I get it a bit better now but I basically almost exclusively used the SMG throughout the game (huge ammo capacity by the end of the game, near 800 rounds) and broke out a heavy pistol for when a mech turned up. This is where I felt they could've included a couple more choices rather than having a starting weapon and one upgrade (maybe a third in some categories). The ammo capacity for the heavy pistol was a joke, and I honestly thought the shotgun felt absolutely useless. At long range (obviously), and up close it still wasn't putting out enough damage to keep your own arse safe.
I feel I've been a bit negative so far, so for the things I like. The new minigames are good, and I like how a lot of content is no longer blocked off because I didn't choose a 'tech' class. Pretty much everyone you meet in this game is really well written and acted. I liked the banter you get whilst wandering around the ship (particularly the two engineers). I like that the achievements have been altered so there's less grinding involved, and that they've removed the "Complete the majority of the game with ______" ones so I can switch out characters as I feel like it. The health system was fine by me, and if we're having ammunition being included as part of the lore then health should be too (codex says armoured suits distribute medi-gel to "patch up" wound sites). The interruption system is also a great idea (and some of the Renegade options shown in that video on the first page are great).
Oh, and Mordin singing Gilbert and Sullivan.
I was worried for the first 20 hours or so that the game was going to be way too short. It just felt like it was building up to a climax way too quickly, although it turns out there was plenty of meat to the game so it was a bit of a non-issue in the end.
I pretty much agree with what Jep said on the first page:
I've also got to voice my agreement with Thrawn about the deaths of characters. Very rarely have I been this involved in wanting every member of my crew to make it through the end (which I managed), and it's started me wondering just how different the finale goes if you have few people (or even no-one) loyal to you. Perfect example of caring for the crew in this game was during Tali's loyalty mission. Using the Paragon interruption to give her a hug after finding her father was a very touching moment.
Oh, and if anyone's interested: Zaeed's loyalty does not factor in to getting the "No One Left Behind" achievement.

I was also thinking about Joker using the assault rifle. I guess Cerberus gave him some medical help with his condition while he was working for them since he was pretty mobile, but I can't remember what he said (if anything) about his condition affecting his mobility in the first game.
